- Applicants must attend orientation seminar every Tuesday at 9:00am.
- Fill up 12 copies (4 sets) of Application Form and Service Contract.
- Submit the filled up application form to the Customer Service.
- Assessment or Evaluation of the Area.
- Approval of the Contract.
- Purchase of Pipes and Fittings.
- Payment.
- Installation.

- Attestation/Verification of Housing Developer duly Notarized
- Certificate of Completion
- Certificate from COWD Authorizing MFWD to install Water Service Connection **
- Certificate of Residency **
- Picture of 2 Empty Drums
- 2 Valid ID’s
- Proof of Ownership (Land Title/Deed of Sale)
